気管切開により救命し得た流行性耳下腺炎に伴う喉頭浮腫例

A case of laryngeal edema requiring urgent tracheostomy due to mumps virus infection.

Ⅰ　はじめに

　流行性耳下腺炎はムンプスウイルスによる感染症であり，主に小児期に好発し，典型的には耳下腺の腫脹，疼痛や発熱を生じる。また，感音難聴や髄膜炎，膵炎，精巣上体炎，卵巣炎などの合併症も生じることがある。咽喉頭浮腫の合併は稀であるが，気道にかかわる合併症であり注意を払わなければならない。われわれは，気管切開により救命し得た流行性耳下腺炎による喉頭浮腫の1成人例を経験したので報告する。

　A 29-year-old woman was admitted to our hospital by ambulance because of dyspnea. Her parotid grands and submandibular grands were swallen bilaterally. Emergency intubation was unsuccessful because of extreme laryngeal edema and she underwent tracheostomy immediately. After admission,her serum mumps IgM,IgG titers were positive. In case of submandibular grand involvement with mumps,we should pay attention to laryngeal edema.
