眼瞼脂腺癌リンパ節転移に対する治療法についての検討

Clinical study on treatment for lymph node metastasis of sebaceous carcinoma of the eyelid

Ⅰ　はじめに

　脂腺癌はマイボーム腺や眼瞼皮膚および眉毛部の脂腺から発生する稀な上皮性悪性腫瘍で，多くが眼瞼に生じる。原発巣に対する手術は眼科で治療される場合が多いが，17～33％と比較的高頻度に耳下腺または頸部リンパ節転移をきたし^1,2），われわれ耳鼻咽喉科医もその取り扱いについて知っておくべき疾患である。今回われわれは，眼瞼脂腺癌の局所治療後に耳下腺および頸部リンパ節に転移を認めた5症例を経験したので，術式を中心とした治療法について文献的考察を加えて報告する。

　Because sebaceous carcinoma is a rare malignant tumor that usually arises in the eyelid,errors or delays in diagnosis are common. However,this tumor must be considered as an aggressive neoplasm with potential for regional and distant metastasis. The rarity of this tumor also makes it difficult to assess the patterns of lymphatic spread and to determine the optimal extent of treatment. We presented five cases of regional lymph node metastasis of sebaceous carcinoma of the eyelid after local treatment. The results suggested that we should take lymphatic flow into consideration in the management of regional lymph node metastasis of sebaceous carcinoma of the eyelid.
