マイクロデブリッダーが有用であった成人型多発性喉頭乳頭腫の1症例

Microdebrider resection of multiple laryngeal papillomas in adult

Ⅰ．はじめに

　喉頭乳頭腫は喉頭良性腫瘍のなかで最も頻度が高く^1,2），そのなかに再発を繰り返す難治例が存在する^3）。治療法として，わが国ではCO₂レーザーを用いた手術療法が一般的であるが，欧米ではマイクロデブリッダーによる切除が導入されつつある^4～7）。今回われわれは，成人型の多発性喉頭乳頭腫症例に対して，マイクロデブリッダーを用いた腫瘍切除を行い，良好な経過をえたので，喉頭乳頭腫治療におけるマイクロデブリッダーの有用性についての文献考察検討を加えて報告する。

　Surgery is the primary modality for the treatment of laryngeal papilloma. Vaporization using carbon dioxide laser,which allows bloodless procedure,has been considered mainstream in Japan. Recently in the US,powered instrumentation was introduced to the laryngology including the removal of papilloma. We experienced a case with multiple laryngeal papillomas,resulted in a quick and precise resection of 6 papilloma lesions by microdebrider. No recurrence was observed. Under the adequate selection of the case,this instrument can be useful to remove laryngeal papillomas.
