遺伝性甲状腺髄様癌MEN2Bの1例

A case of multiple endocrine neoplasia type 2B with laryngeal mucosal neuromatosis

Ⅰ．はじめに

　甲状腺髄様癌はC細胞由来の癌であり，カルシトニンとCEAを産生する。この腫瘍は全甲状腺悪性腫瘍の約1.5％と少ない^1）が，その約1/3は多発性内分泌腫瘍症（multiple endocrine neoplasia：MEN）2A型や2B型あるいは家族性甲状腺髄様癌の常染色体優性遺伝を有する遺伝性腫瘍であり，その診断や治療には注意を要する^1）。

　MEN2型として一般によく知られているMEN2Aは甲状腺髄様癌，副腎褐色細胞腫および副甲状腺腺腫や過形成を合併するものである。一方，MEN2Bは甲状腺髄様癌，褐色細胞腫および粘膜神経腫（mucosal neuromatosis）を合併するものであり稀な疾患である。MEN2Bの患者は粘膜神経腫のため口唇や舌が肥厚し，またMarfan様の骨格異常を伴うことが知られており，特徴的な体型と顔貌を呈する^2）。粘膜神経腫は生直後ないしは幼少期に口唇，舌縁部，口腔内，眼瞼などに生じることが知られている^3）。

　今回われわれはMEN2Bで喉頭に粘膜神経腫および喉頭肉芽腫を生じていた症例を経験したので報告する。

　The association of medullary thyriod carcinoma,pheochromocytoma,and mucosal neuromatosis has been classified as multiple endocrine neoplasia type 2B（MEN2B）. The presence of mucosal neuromas on the distal tongue,conjunctiva,and through out the gastrointenstinal tract give this clinical syndrome its most distinctive feature. We presented a case of MEN2B with laryngeal mucosal neuromas after intubation.
