頸部リンパ節転移で発見された扁桃オカルト癌

Occult Tonsillar Carcinoma―A Case Report―

I．はじめに

　頸部腫瘤を摘出して病理組織学的検査を行い転移性癌と判明した場合，種々の検査を行い原発巣検索が行われる。しかし，精査を行っても原発巣が発見できず原発不明癌として治療される患者の割合が全頭頸部癌の1～5％^1）を占めている。これら原発不明癌として一次治療を開始された症例のうち，10～30％^2）でのちに原発巣が判明するとされる。最近このような症例で，比較的高率に口蓋扁桃に原発巣が発見されたという報告が散見される^3）。

　今回われわれは頸部転移性扁平上皮癌症例で，原発巣の検索に難渋しながらも，口蓋扁桃の病理組織学的検査で一次治療開始前に原発巣が判明した症例を経験したので報告する。

　A 64-year-old man presented with a complaint of a right upper neck mass which was diagnosed as metastatic squamous cell carcinoma by excisional biopsy. Further examinations for the primary site including panendoscopy of the upper aerodigestive tract,computed tomography of the chest,blind biopsy of the nasopharynx were all negative. Because ipsilateral tonsillectomy revealed an occult tonsillar carcinoma,radiation therapy to the primary site and additional conservative neck dissection were performed. For ten months postoperatively,there was no evidence of recurrence.
