喉頭全摘後の再発癌に対して遊離空腸とDP皮弁で再建を行った1例

Reconstruction of hypopharynx,esophagus and skin of the neck with combined microvascular free jejunal transfer and deltopectoral flap：A case report

I．はじめに

　DP皮弁（deltopectoral flap）は頭頸部外科領域の再建に広く用いられている皮弁で，頭頸部癌の頸部皮膚合併切除の再建や術後に生じた瘻孔の閉鎖に使用されることが多い^1）。一方，遊離空腸移植は現在では主に咽頭・喉頭・頸部食道摘出術に対して用いられ，標準的再建方法の1つとなっている。咽頭・喉頭・頸部食道に加えて頸部皮膚も切除される症例で遊離空腸とDP皮弁を組み合わせることにより一期的に再建した報告は比較的少ない^2）。今回，喉頭癌の喉頭全摘後に皮下に再発を認め，下咽頭，頸部食道，皮膚を合併切除した症例に対して遊離空腸とDP皮弁を組み合わせて再建を行い，良好な結果を得ることができたので，若干の文献的考察を加えて報告する。

　Massive defects of the neck area after combined resection of central compartment and the skin is very difficult to reconstruct. A patient underwent resection of the hypopharynx,esophagus,and skin of the neck for recurrence of squamous cell carcinoma after laryngectomy. The alimentary tract was replaced by free jejunal flap and its surface was covered with deltopectoral flap in a single stage. The deltopectoral flap provides not only well-vascularaized tissue for the defect,but also provides thin and pliable one that is easily contoured to fit the peristomal defect.
