Navigation systemを利用した頭頸部悪性腫瘍手術

Computer-Guided Surgery in Head and Neck Lesions

I．はじめに

　Navigation systemは主に鼻内内視鏡下副鼻腔手術において活用されている。その利用範囲は拡大しているが，頭頸部悪性腫瘍の手術において使用した報告はまだ少ない^1）。

　当科では現在までに7症例の頭頸部悪性腫瘍に対してナビゲーション支援手術を施行し，その有効性を実感している。今回，その中から代表的な症例を選んで呈示し，その有効性や問題点について検討を加えて報告する。

　Recently we started computer-guided surgery with an optical-tracking navigation system（Stealth Station^TM）in head and neck lesions. Seven patients presenting with head and neck cancer underwent surgery under the use of this system from October 2000 to June 2003. The authors describe three cases among them and evaluate the advantages of the system for paranasal and skull base legions. Surgeons with the assistance of the system can identify the target structures in an accurate and reliable manner. The main disadvantage of the system is its high cost.
