診断に苦慮した上顎明細胞型粘表皮癌の1例

A case of the mucoepidermoid carcinoma clear cell variant in the maxillary sinus

I．はじめに

　鼻，副鼻腔に発生する上皮性悪性腫瘍は扁平上皮癌が60％^1）～80％^2）で，粘表皮癌は約1％といわれている^3～5）。一般に上顎粘表皮癌は術前診断が困難なことが多く，本邦では術前診断が扁平上皮癌であったという報告が多い^4）。また，粘表皮癌は唾液腺に好発し，唾液腺のない上顎洞内に発生することは稀である。上顎洞に腫瘍が認められる場合，ほとんどが進行して鼻腔内に広がったり，上顎骨を破壊した状態で発見されるため，上顎洞内に原発したものかどうか確定することは困難なことが多い^6）。

　今回われわれは，上顎粘表皮癌に含まれる稀な明細胞型の1例を経験したので報告する。

Mucoepidermoid carcinoma is rare in the nose and sisuses,especially clear cell variant is very rare. Clear cell variant mucoepidermoid carcinoma is difficult to be differentiated from renal cell carcinoma and others. A 69-year-old woman was complaining of right nasal obstruction. MRI showed a blood boil like mass. Embolization of the maxillary artery was performed before operation to prevent a massive bleeding during operation,and the tumor was removed by transoral and endscopic surgery. Finally,a mucoepidermoid carcinoma was determined as clear cell variant by immunohistochemical examinations.
