糖尿病を合併した原田病のステロイド治療

Systemic corticosteroid treatment for Harada's disease associated with diabetes mellitlls

（展示113）　糖尿病を合併した原田病の3例を経験した。眼科初診時から糖尿病に罹患していた2例では，ステロイド大量投与とインスリン治療および食事療法を併用した。治療経過中に糖尿病を発症した1例は，スルホニルウレア剤投与と食事療法で治療した。いずれの症例もステロイドの大量投与を行ったが，原田病の遷延化はなく，糖尿病網膜症の悪化や発症もなかった。ステロイド治療前に糖尿病精査を十分に行い，原田病に対するステロイド投与と糖尿病の治療を同時に行えば，原田病，糖尿病ともに予後は良好であると考えられた。

We treated three cases of Harada's disease associated with diabetes mellitus.Two patients had diabetes atthe onset of Harada's disease.They were treated with high dosis of systemic corticosteroid.insulin and diet therapy.Thethlrd case developed diabetes during the course of treatment.This patient was treated by systemic sulfonylurea,diettherapy and tapering of systemic corticosteroid.All the cases resulted in resolution of Harada's dlsease and showed noaggravation of diabetic retinopathy.These cases illustrate the efficacy of systemic corticosteroid for Harada's disease indiabetic patients when performed under the advice of diabetes specialists.
