A 55-year-old male developed acute toxic epider-mal necrolysis (TEN) 5 weeks after surgery for lung cancer. Systemic antibiotics seemed to have triggered the TEN. In both eyes, large areas of the corneal epithelium became either necrotic or absent during the acute phase. The secondary cornea wascovered by dense scar tissue. Skin biopsy during the acute phase showed findings similar to those in graft-versus-host reaction (GVHR). We suspected that the severe corneal epithelial injury in this case was due to a process analogous to GVHR. The ocular lesions in this case closely simulated those in Stevens-Johnson syndrome, suggesting that GVHR is involved in the clinical manifestation of this syndrome.
