六神丸を誤って眼に入れ眼障害をきたした1例

A case of ocular injury by mistaken topical use of Rokusingan

　77歳女性が強心剤である六神丸の顆粒を点眼薬と誤認して右眼に入れ，眼痛が生じた。2時間後の所見として，点状表層角膜炎，角膜上皮びらん，結膜浮腫，毛様充血，前房内微塵が右眼にあり，矯正視力は0.5であった。右眼を洗眼し，結膜嚢内の六神丸顆粒を除去したが，デスメ膜皺襞と角膜浮腫はさらに増強した。副腎皮質ステロイド薬の点眼と静注を行い，以後角膜の混濁浮腫は減少し，7日目にデスメ膜皺襞は消失した。家庭薬である六神丸が点眼薬に似た容器に入っていたための事故であり，注意が望まれる。

A 77-year-old female put granules of Rokusingan, a cardiac stimulant, into her right eye by accident. When seen 2 hours later for ocular pain, her right eye showed superficial punctate keratitis, segmental corneal erosion, folds in Descemet membrane, conjunctival edema, ciliary injection and aqueous cells. Thorough washout of the con-junctival sac and removal of granules, folds in Descemet membrane and corneal edema increased further. Following systemic and topical corticosteroid, corneal edema gradually subsided with disappearance of folds in Descemet mem-brane one week later. The accident was caused because Rokusingan was contained in a container simulating that for eyedrops. Due precaution is necessary to avoid such an accident.
