A 52-year-old woman was referred to us for papilledema in her right eye. Her corrected visual acuity was1.5 in either eye. Funduscopy showed hyperemic and swollen optic disc in her right eye. A dilated vessel was present inthe disc area which appeared to be an optociliary shunt vessel (OSV) . The dilated vessel had increased in number whenseen one year later. Fluorescein angiography showed them to be true OSVs and to be 7 in number. Seven years from herinitial visit, her right visual acuity was reduced to no light perception. The OSVs became 3 in number. Computed tomography (CT) showed swelling of the optic nerve simulating an hourglass. Transcranial extirpation of the optic nerve tumor ledto the diagnosis of optic nerve meningioma. This case illustrates that OSVs may develop in optic nerve meningioma andthat the OSVs may undergo changes in angioarchitecture over a long period.
