Acinetobacter calcoaceticusが分離された眼科感染症の検討

Clinical studies of ocular infections from which Acinetobacter calcoaceticus was isolated

　過去8年間に新潟大学医学部眼科感染症クリニックで，Acinetobacter calcoaceticusが分離された7症例を検討した。内訳は，急性結膜炎1例，慢性結膜炎2例，急性涙嚢炎1例，角膜びらん1例，角膜潰瘍2例である。入院患者4例，外来患者が3例である。眼科的には兎眼や鼻涙管狭窄，全身的には糟尿病，肝炎，中枢神経障害などの基礎疾患が多く，易感染性宿主の状態にあった。治療として，oOfloxacinの点眼と，imipenem, levofloxacinの全身投与を行い，症状が改善した。分離されたA．calcoaceticusはminocycline, ofloxacinに対して高感受性を，ペニシリン系，セフェム系には低感受性を示した。

We reviewed 7 cases of ocular infection from which Actinobacter calcoaceticus was isolated. They seen at theInfection Clinic of Department of Ophthalmology, Niigata University during the past 8 years. The series comprised acuteconjunctivitis 1 case, chronic conjunctivitis 2 cases, corneal ulcer 2 cases and 1 case each of acute dacryocystitis andcorneal erosion. Four cases were inpatients and 3 were outpatients. All had underlying disorders including:lagophthalmos,stenosis of nasolacrimal duct, diabetes mellitus, hepatitis or central nerve disorder. They were effectively treated withtopical ofloxacin and systemic imipenem and levofloxacin. Most strains of isolated A. calcoaceticus were highly sensitiveto imipenem, minocycline and ofloxacin and were less sensitive to penicillins and cephems.
