房水漏出を認める濾過胞に対する遊離結膜弁移植

Use of free conjunctival autograft in the treatment of leaking filtering blebs in glaucoma

　緑内障濾過手術後に濾過胞に房水漏出が生じた3例3眼に対して，同側から採取した遊離結膜弁を移植した。2眼は過去にマイトマイシンC併用の線維柱帯切除術を受け，1眼では手術内容は不明であった。濾過胞炎の既往が2眼にあった。眼圧は3眼とも4mmHg以下であった。遊離結膜弁移植術後，眼圧は6から16mmHgに維持され，濾過胞消失や房水漏出はなかった。房水漏出のある濾過胞に対して遊離結膜弁移植は有効である。

We repaired leaking bleb in 3 eyes of 3 patients using a free conjunctival autograft obtained from the same eye. Two eyes had received trabeculectomy with adjunct use of mitomycin C. No detailed information was obtained in the other eye regarding the past glaucoma surgery. Two eyes had episodes of blebitis. The intraocular pressure (IOP) was below 4mmHg in all the 3 eyes. After surgical repair, all the eyes showed resolution of bleb leak with the IOP maintained between 6 and 16mmHg. There was no instance of disappearance of functioning bleb or aqueous leakage. These cases illustrate the efficacy of free conjunctival autograft for leaking filtering blebs.
