遊離結膜弁移植による緑内障手術での濾過胞形成

Creation of a filtering bleb using free autologous conjunctival graft in glaucoma surgery

　複数回の手術の既往がある54歳女性の開放隅角緑内障に対して7回目の濾過手術を行った。すでに結膜が菲薄化し，以後の手術では房水漏出の可能性が高いと考えられたので，失明していた他眼からの遊離結膜弁移植を用い，マイトマイシンCを併用した。術後に房水漏出はなく，眼圧コントロールも良好で，他眼にも著変なく経過した。当初から房水漏出が予想される再手術で，計画的な他眼からの結膜弁移植が有用であることを示す症例である。

A 54-year-old woman received a seventh filtering surgery in her left eye for refractory open-angle glaucoma. The bulbar conjunctiva had become very thin suggesting a high risk of bleb leak after conventional filtering surgery. Therefore, a free conjunctival piece was excised from her right blind eye to provide a support to the filtering bleb. Mitomycin C was used adjunct to the seventh filtering surgery. A well- developed filtering bleb resulted with no aqueous leakage. The intraocular pressure has been well controled with topical timolol. This case illustrates the usefulness of free autologous conjunctival tissue transplant in treating thin conjunctival bleb over the filtering bleb.
