交代性上斜位の合併が疑われた外傷性上斜筋麻痺の1例

A cace of traumatic superior oblique palsy with dissociated vertical deviations

　6歳女児が頭位異常と右眼の眼球運動障害で受診した。1歳時に自転車から転落して頭部を打撲し，その夜から頭部の左傾が起こっている。交代性上斜位が併発した右眼外傷性上斜麻痺と診断した。上斜筋麻痺による上下偏位と回旋偏位が交代性上斜位を誘発したと推定された。

A 6-year-old girl presented with abnormal head posture and abnormal movement of the right eye. She had suffered from head injury when she was one year old. Head tilting had been persistent since the day of injury. We diagnosed her as traumatic superior oblique palsy in the right eye with alternating dissociated vertical deviations. Her dissociated vertical deviations were apparently induced by vertical and tosional disparities due to superior oblique palsy.
