交代性上斜位へのFaden手術の検討

Faden operation for dissociated vertical deviation

　交代性上斜位（DVD）の患者11名13眼にFaden手術法を試み，良好な結果が得られた。手術眼の術前の平均垂直斜視角19.5prismdiopter （△）が，術後平均垂直斜視角10.8△となり，平均偏位率は26.6％と減少傾向にあった。手術眼にて無効と判定された症例においても非手術眼にて効果が認められた。このことは，手術眼のみでなく非手術眼にもFaden手術は効果が期待でき，DVDの斜視角減少に有効な手術法と思われた。

We treated 13 eyes in 11 patients with Faden operation to correct dissociated vertical deviation.As the standard procedure, the superior rectus muscle was sutured to the sclera 12, 13 and 14mm posterior to the insertion. The superior rectusmuscle was also recessed 2 to 4mm from the inser-tion. The cases were followed up for 6 months to 5 years, average 18.2 months. The operation was beneficial in 11 eyes, 9 patients. Vertical deviation was also reduced in the fellow eye in 2 cases.
