We evaluated the gonioscopic features of 341 eyes of 196 cases with pseudoexfoliation syndrome. Age-matched 360 eyes of 180 cases with cataract served as control. Angle pigmentation at 6 o'clock position was significantly more intense in the affected eyes than in controls (p < 0.001) and particularly in eyes with glaucoma. In 89 unilaterally affected cases, angle pigmentation was significantly more intense in the affected than in the fellow eyes (p < 0.001) . Angle pigmentation in the unaffected fellow cases was more intense than in controls (p <0.001) . The incidence of Sampaolesi line was greater in eyes with pseudoexfoliation than in controls (p < 0.001) and was less in eyes with ocular hypertension. Nerrow chamber angle was more frequent in affected eyes than in controls. This feature was more apparent in eyes with glaucoma. The findings show that eyes with pseudoexfoliation are characterized by hyperpigmentation and Sampaolesi line in the chamber angle and that both features may be involved in pseudoexfoliation glaucoma.
