Sjögren症候群とその他のドライアイにおける涙液表面観察像の比較

Comparison of surface appearance of tear film in Sjögren syndrome and other dry eyes

　Sjögren症候群29例49眼とその他のドライアイ21例39眼について，涙液表面のスペキュラー像を検討した。非接触型の涙液表面観察装置を用いてSjögren症候群，その他のドライアイ，健常眼13例22眼を観察し，5つのGrade （Gradel：干渉色が灰色で分布が均一，Grade2：干渉色が灰色だが分布が不均一，Grade3：干渉色が2色以上で分布が不均一，Grade4：干渉色が多彩，Grade5：角膜表面が露出し，透見可能）に分類した。各群においてGrade分布を比較すると，Sjögren症候群，その他のドライアイ，健常眼の順に前者ほどGradeの高い例が有意に多かつた（各p＜0.0001）。今回の検討により，涙液表面の観察は，健常眼とドライアイ，特に健常眼とSjögren症候群を鑑別する方法として有用であると考えられた。

We observed the specular images of the tear film in the central cornea in 49 eyes of sjögren syndrome, 39 of dry eye and 22 of healthy volunteers. A noncontact observation and recording system was used. The findings were classified into five grades. Grade 1 : grayish color and uniform distribution. Grade 2 : grayish color and nonuniform distribution. Grade 3 : a few colors and nonuniform distribution. Grade 4 : many colors and nonuniform distribution. Grade 5 : corneal surface partially exposed. Eyes of sjögren syndrome showed significantly higher grades than in dry eyes not related to sjögren syndrome (p ＜ 0.0001) . The findings show the usefulness of observation of the tear film surface in differentiating among sjögren syndrome, other dry eyes and healthy eyes.
