外傷に起因すると考えられた水晶体脱臼を合併したアトピー性皮膚炎症例を報告した。症例は62歳男性。幼少からアトピー性皮膚炎に罹患。皮膚炎増悪時に顔面の掻痒が強く,激しく顔面を殴打していた。右眼視力低下を自覚し眼科受診,両眼に水晶体脱臼がみられ右眼は硝子体内に完全脱臼,左眼は亜脱臼。右眼は硝子体腔内で落下水晶体を切除吸引,眼内レンズ2次縫着,左眼は水晶体全摘と眼内レンズ縫着を行い,術後経過は良好である。本例の水晶体脱臼は,眼球の強い打撲によって加齢のため比較的脆弱なチン小帯が断裂し生じたものと考えられた。
A 62-year-old male presented with bilateral luxation of the crystalline lens. He had been suffering from atopic dermatitis since childhood. He habitually slapped his face violently to relieve the severe itching. The lens was dislocated into the vitreous in the right eye and was subluxated in the left. The luxated lens in the right eye was removed by vitrectomy. The lens in the left eye was removed through limbal incision. Both eyes underwent intraocular lens implantation. The lens luxation appeared to have been induced by slapping the itching eye.
