角膜トレパンを用いた白内障手術創

Scleral incision with a corneal trephine in cataract surgery

　無縫合白内障手術のひとつの問題点として術後倒乱視化がある。多くの術式において角膜接線と直交する方向に強膜創の離開が生じることが原因のひとつと考えられる。そこで筆者らは，角膜トレパンにて円形の強膜創を作成し，その術後乱視経過を従来の切開創と比較検討した。直線切開に比べ術後乱視幅は減少し，3か月の経過で36例中10例（28％）に直乱視化を認めた。この結果から，横方向のベクトルを考慮した強膜切開が有効である可能性が示唆された。また常に再現性のある切開にするためこのような器具を使用することは意義があると思われた。

One of the problems of sutureless cataract surgery is postoperative against-the-rule ker-atometric astigmatism. Scleral wound dehiscence is its major cause. We used an electrically driven 4-mm trephine to incise the sclera for cataract surgery in 36 eyes. The semicircular incision resulted in lesser astigmatism when compared with conventional linear incision. At 3 months postoperatively, the preoperative astigmatism turned into with-the-rule astigmatism in 10 of 36 eyes, 28%. The present procedure appeared to be of value in reducing the postoperative keratometric astigmatism.
