One of the problems of sutureless cataract surgery is postoperative against-the-rule ker-atometric astigmatism. Scleral wound dehiscence is its major cause. We used an electrically driven 4-mm trephine to incise the sclera for cataract surgery in 36 eyes. The semicircular incision resulted in lesser astigmatism when compared with conventional linear incision. At 3 months postoperatively, the preoperative astigmatism turned into with-the-rule astigmatism in 10 of 36 eyes, 28%. The present procedure appeared to be of value in reducing the postoperative keratometric astigmatism.
