糖尿病黄斑症における光凝固後の中心視野

Central visual field after laser photocoagulation for diabetic maculopathy

　糖尿病黄斑症を局所性，びまん性，類嚢胞黄斑浮腫（cystoid macuiar edema：CME）に分類し^1），局所性には局所にびまん性とCMEには格子状に，黄斑部を光凝固した。光凝固前後の中心視野をオクトパス視野計のプログラム38を用い測定し光凝固の黄斑部機能への影響を検討した。

　中心感度，平均感度は，局所性群とびまん性＋CME群のどちらの群においても，術前と術後との間に有意差はなかった。中心感度の改善および不変例は，局所性群で11眼中9眼，びまん性＋CME群で11眼中8眼認められた。平均感度は，2群とも改善例はなく不変例が9眼あった。

　以上より黄斑症の光凝固は網膜感度において黄斑全体の機能を維持できると思われた。

We evaluated the macular function in 22 eyes with diabetic maculopathy treated by focal or grid laser photocoagulation. The series comprised 11 eves with focal macular edema and 11 eyes with diffuse macular edema or cystoid macular edema. Macular function was assessed by program 38 of octopus perimeter. There was no difference before and after photocoagulation in foveal and mean sensitivities between the two groups. The foveal sensitivity improved or remained unchanged in 9 eyes with focal macular edema and in 8 with diffuse macular edema or cystoid macular edema. The mean sensitivity remained unchanged in 9 eyes in either group. The finding suggests the efficacy of photocoagulation in maintaining the overall macular function in diabetic maculopathy.
