糖尿病網膜症での輪状増殖病変の形成過程

Process of ring-shaped proliferation in diabetic retinopathy

　増殖性糖尿病網膜症で生じた輪状増殖性病変の形成過程を，20例24眼で観察した。初診時には24眼全例で硝子体が未剥離であった。光凝固開始から平均4.6か月後に，全例に血管アーケードの外側の中間周辺部で，全周もしくは部分的な硝子体剥離が起こった。後極部では黄斑前の後部硝子体皮質前ポケット（ポケット）に一致して，ゲルが眼底から分離していた。後部硝子体膜の剥離（PVD）は19眼では，後極部には全経過を通じて起こらなかったが，そのうち6眼で牽引性網膜剥離を合併した。5眼ではトランポリン状の浅いPVDが起こった。

　この輪状増殖病変は，18眼では後極部を底としたすりばち状の不完全なPVDが生じ，それを足場にして発達した新生血管線維膜で形成された。この段階で成長する新生血管はおもに視神経乳頭と耳側血管アーケード由来であった。

　輪状病変の形成には，血管線維膜以外に，黄斑前のポケットの輪状の輪郭が，硝子体出血や硝子体ゲルの混濁で顕性化すること（4眼）や，ポケットの外縁のゲルが線維化すること（5眼）も関与していることが，今回の検索で明らかになった。

We evaluated the process of development of ring-shaped vitreous lesion in 24 eyes with prolifer-ative diabetic retinopathy. None of the 24 eyes showed posterior vitreous detachment (PVD) nor fibrovas-cular proliferation initially. PVD developed outer to the temporal vascular arcades after a mean of 4.6 months following start of photocoagulation. The posterior fundus showed no PVD in 19 eyes, shallow trampoline-like PVD in 5 and tractional retinal detachment without PVD in 6. The ring-shaped proliferation thus followed fibrovascular proliferation along the external border of partial, cone-shaped PVD. In some eyes, opacification of ring-shaped contour of premacular liquefied pocket due to vitreous hemorrhage or condensation of vitreous gel also contributed to premacular ring-shaped lesion.
