背景要因が異なる多発性後極部網膜色素上皮症の3例

Three cases of multifocal posterior pigment epi-theliopathy with differing background factors

　多発性後極部網膜色素上皮症（MPPE）の発症に関して，全身的に異なる背景要因をもった3症例を報告した。症例1は44歳の男性で，トキソプラズマ抗体価，好酸球，血中IgEなどが高値であった。症例2は大動脈弁閉鎖不全・狭窄，慢性腎不全，糖尿病を有する53歳の女性で，右眼眼内レンズ移植術約1か月後に発症した。腕網膜時間，網膜内循環時間の延長を認めた。症例3は若年期より高血圧，腎障害を有する39歳の男性で，滲出斑周囲の螢光色素の脈絡膜充盈の著しい遅延を認めた。MPPEの発症には免疫アレルギー脈絡膜循環障害，腎障害なども関与する可能性があると考察した。

We observed unilateral multifocal posterior pig-ment epitheliopathy in 3 cases: 2 males aged 44 and 39 years, and a female aged 53 years. One case showed elevated titers for toxoplasma, serum IgE and eosinophiles. Another case showed systemic hypertension since the age of 18 years and ne-phropathy. Fluorescein angiography showed delayed filling of choriocapillaris around exudative lesions. The third case showed regurgitation and stenosis of aortic valve, chronic renal insufficiency and diabetes mellitus of recent onset. It appeared that immunological disorder, impaired choroidal chrculation and nephropathy served as background for the multifocal posterior pigment epitheliopathy.
