先天白内障術後の屈折変化

Changes in refraction following surgery for congenital cataract

　生後2歳以内に先天白内障手術が行われた症例の屈折異常値について，生後5歳までの経年変化について検討した。生後2歳までは片眼性症例，両眼性症例ともに等量の屈折値の変化が認められた。しかし，2歳から5歳までの経年変化は，片眼性症例が2倍以上の屈折変化を示し，片眼性症例により強い正視化，近視化が生じていた。その経年変化量の差を生じた要因は手術操作ではないと思われ，2歳以後に手術された片眼性6症例を加え，視力予後の程度で分類し生後3歳から5歳までの術後屈折異常値について検討すると，視力予後不良群のほうが有意に低い屈折異常値を示し，術後屈折値の変化は視力予後と強い関連があるものと思われた。

We evaluated the course of aphakic refraction following surgery for congenital cataract perform-ed before 2 years of age. There was no difference before 2 years of age between 8 infants with unilat-eral and 15 with bilateral cataract. Up to 5 years of age, unilateral cases showed more manifest ten-dency to emmetropia or myopia. When the post-operative refractive value was evaluated against visual acuity at 5 years of age, eyes with better visual acuity tended to be more hyperopic than eyes with poor visual acuity. Conversely, postoper-ative refractive power seemed to serve as an indica-tor for visual prognosis.
