　大きな網膜下出血や硝子体出血をきたした重症の網膜下血腫型加齢黄斑変性症5例5眼に対して硝子体手術を行い，網膜下血腫および増殖組織除去を施行した。手術は網膜下病変の除去に加え，眼内光凝固，強膜赤道部輪状締結術，SF₆，またはシリコンオイル注入を行った。術後，網膜剥離は消失し網膜は平坦となったが，病変除去部の網膜色素上皮は高度に障害された。その障害部での網膜色素上皮の動態と視機能の変化について検討を行った。経過観察期間中，アムスラーチャートで中心暗点の縮小を認め，検眼鏡的ならびに螢光眼底撮影で，無色素網膜色素上皮の再生を思わせる所見が得られた。

We treated 5 eyes with subretinal hemorrhage and fibrovascular tissue associated with age -related macular degeneration. We performed vitrectomy, retinotomy, removal of subretinal hematoma and fibrovascular tissue, endophoto-coagulation, scleral encircling and SF₆ or siliconeoil implantation as standard procedure. While hemorrhagic retinal detachment disappeared after surgery, the retinal pigment epithelium was highly damaged by the subretinal manipulation. During the follow up of maximally 17 months, central scotoma decreased associated with possible regen-eration of nonpigmented retinal pigment epithelium by ophthalmoscopy and fluorescein angiography.
