A 44-year-old male underwent extracapsular cataract extraction and intraocular lens implanta-tion. postoperative nonbacterial endophthalmitis developed 8 months after surgery. It failed to respond to medications with antibiotics or corticos-teroids. Deterioration of electroretinogram (ERG)ensued with visual acuity reduced to light percep-tion. Pars plana vitrectomy revealed a ring-shaped mass around the posterior lens capsule, suggesting a phacoanaphylactic nature of the endophthalmitis. The visual acuity improved to 0.5 after vitrectomy with improvement in the amplitude of A-wave and B-wave of ERG. However, the oscillatory poten-tials remained abolished, suggesting persistence of retinal damage.
