重篤な網膜機能障害が示唆された白内障術後の無菌性眼内炎の1例

A case of aseptic endophthalmitis after cataract surgery with severe retinal damage

　薬物療法では改善のみられなかった白内障術後の持続性眼内炎に硝子体手術を施行した。硝子体沈渣と前房水の培養結果は陰性であり，術中認められた膜様組織から水晶体過敏性眼内炎が最も疑われた。術前視力は光覚弁まで低下し，ERGは著しく減弱していたが，硝子体手術後には視力は0.5まで改善しERGのa波，b波の振幅はほぼ回復した。しかし律動様小波（OP波）は術後も回復せず，網膜に不可逆性の障害が残ったと考えられこのような無菌性眼内炎でも網膜に障害を与える可能性があることが示唆された。

A 44-year-old male underwent extracapsular cataract extraction and intraocular lens implanta-tion. postoperative nonbacterial endophthalmitis developed 8 months after surgery. It failed to respond to medications with antibiotics or corticos-teroids. Deterioration of electroretinogram (ERG)ensued with visual acuity reduced to light percep-tion. Pars plana vitrectomy revealed a ring-shaped mass around the posterior lens capsule, suggesting a phacoanaphylactic nature of the endophthalmitis. The visual acuity improved to 0.5 after vitrectomy with improvement in the amplitude of A-wave and B-wave of ERG. However, the oscillatory poten-tials remained abolished, suggesting persistence of retinal damage.
