外頸動脈起源の眼動脈を有し，一過性黒内障を頻発した外頸動脈狭窄症の1手術治験例

A Case of Amaurosis Fugax with Anomalous Origin of the Ophthalmic Artery and Atheromatous Stenosis of the External Cartotid Artery

I．はじめに

　一過性黒内障は他の内頸動脈系TIAと同様に，その原因が頸部頸動脈狭窄性病変に存することは古くから知られており，またそれは，頸部頸動脈血栓内膜剥離術のよい適応となることが少なくない，一方，近年，一過性黒内障が同側の内頸動脈閉塞例にみられることが注目され，外頸動脈狭窄性病変がその病因として論じられている^1,2,4-7）．しかしながら，その多くは内頸動脈閉塞後の側副血行路として機能していた外頸動脈-眼動脈系の虚血症状としての一過性黒内障であり^1,2,4-9），眼動脈の分岐異常に起因すると思われるものはWeinberg（1981）の1例^17）のみで，本邦での報告例は見当らない．著者らは最近，外頸動脈起源の眼動脈を有し，一過性黒内障を呈し，外頸動脈起始部の狭窄がその原因として考えられた極めて稀な1例を経験した．外頸動脈狭窄と一過性黒内障の関係につき多少の考按を加え報告する．

Amaurosis fugax associated with anomalous originof the ophthalmic artery without occlusion of internalcarotid artery is very rare. We are reporting such acase of amaurosis fugax with anomalous origin of theophthalmic artery and atheromatous stenosis of theexternal carotid artery.

A 49-year-old-man was referred to our neurosurgicalclinic because of transient monocular blindness. Thepatient was diagnosed as carotid TIA and was treatedwith aspirin and dipyridamole. In spite of this treat-ment, he experienced further transient dimness ofvision in the right eye, several times during a periodof one month.
