A 65-year-old male suffering from left MLF syn-drome accompanied by transient hallucinose pedon-culaire caused by the midbrain infarction is report-ed. In the first night of his admission with MLF syndrome, he complained of sleeplessness followed by transient visual hallucination. The hallucination was vivid and colored scene in which many stran-gers were seen around the patient, and he was well oriented without dreadfulness at that time. A small infarction in the left tegmentum was revealed by MR imaging, which was supposed to cause the hallucination.
