Abstract Purpose:To report a case of polypoidal choroidal vasculopathy showing significant improvement after oral administration of cabozantinib.

Case:A 69-year-old man was referred to our department for macular degeneration with exudative changes in the left eye.

Findings and clinical course:The patient was diagnosed with polypoidal choroidal vasculopathy by ophthalmologic examination. However, because he was undergoing treatment for other systemic diseases, we could not conduct ophthalmic treatment. Seven months after the first visit, aflibercept injection was administered. Before the second treatment, the patient was hospitalized with compression fractures and ophthalmic treatment was discontinued. Eight months after the initial aflibercept injection, the patient revisited our department and worsening exudative changes were observed. The patient was also diagnosed with recurrent prostate cancer and oral administration of cabozantinib was initiated. One month after the first dose of cabozantinib, the exudative changes associated with polypoidal choroidal vasculopathy had improved. Subsequently, the patient was transferred to another hospital for best supportive care, and ophthalmic follow-up was discontinued.

Conclusion:cabozantinib, an oral inhibitor of VEGF receptors, may be effective in regressing polypoidal choroidal vasculopathy lesions.
