Abstract Purpose:To report a case of sympathetic ophthalmia after vitrectomy for endophthalmitis caused by bleb-related infection.

Case:A 65-year-old man who underwent trabeculectomy for primary open-angle glaucoma in both eyes. Fifteen months after surgery, the patient was diagnosed with endophthalmitis caused by bleb-related infection in the left eye and underwent pars plana vitrectomy with silicone oil tamponade and vancomycin and ceftazidime injections. Visual acuity in the left eye became light sense. One month after vitrectomy, the patient presented with acute visual acuity loss in the right eye. The right eye showed inflammation in the anterior chamber, serous chorioretinopathy, and peripheral choroidal detachment. Optical coherence tomography showed marked thickening of the choroid. Fluorescein angiography indicated inflammatory findings in the optic disc and choroid. Indocyanine green angiography showed leakage from the choroidal vessels. Sympathetic ophthalmitis was diagnosed based on ocular findings and a history of postoperative endophthalmitis in the right eye. Steroid pulse therapy resolved the serous retinal detachment, and the visual acuity in the right eye improved to 0.8, 11 months after the start of treatment.

Conclusions:We experienced a case of sympathetic ophthalmia after bleb-related endophthalmitis. Uveal inflammation caused by a combination of factors such as endophthalmitis, vitrectomy, and silicone oil tamponade might cause sympathetic ophthalmia.
