Abstract Purpose:This study implemented patient and public involvement(PPI)with the iPhone application for hay fever research, AllerSearch®, by adopting novel and diverse perspectives to establish a robust foundation for interactive research.

Material & Methods:PPI members were recruited from November 2019 to January 2020 through a dedicated website. Periodic opinion exchange meetings were held from February 2020 to September 2020 to collect diverse opinions and suggestions expressed by PPI contributors at the meetings. After each meeting, bilateral evaluations of the meeting were collected both from the PPI contributors and the researchers. The comprehensive and multifaceted opinions compiled through PPI were incorporated into the smartphone app to establish an active feedback loop fed by the constantly interactive research infrastructure.

Results:During this study, three PPI members(one male and two females)were recruited and exchange meetings were held five times. Based on the results of the bilateral evaluations of the exchange meetings, 93 items were added or changed both in the research plan and the survey questionnaire of the smartphone app. From the direct feedback from the exchange meetings, frequent opportunities for PPI participants to contribute were ensured, along with clarification of the roles of PPI sought out by the research group. Additionally, in March 2020, a website was created to allow public viewing of the real-time outcomes of the users' hay fever preventative activities collected by the smartphone app. In August 2020, a new, PPI-implemented smartphone app for hay fever was released.

Conclusion:This study marks the first research on clinical smartphone apps on hay fever in Japan, which implements PPI throughout its timeline from research and development to the publication of research results. By taking advantage of the various distinctive perspectives offered by PPI members, a step was taken toward actualizing a foundation for a truly interactive research environment. These results can contribute to the effective promotion of future research and establishment of a social construct that enables PPI efforts in various fields.
