Abstract Purpose:To report a case of necrotizing scleritis apparently due to bacterial infection and autoimmune mechanism.

Case:A 75-year-old male presented with increasing pain and impaired vision in the right eye since 2 weeks before. He had received pterygium surgery in the right eye 10 years before.

Findings and Clinical Course:Corrected visual acuity was 0.6 right and 0.9 left. The right eye showed scarring and calcification supposedly due to pterygium surgery. The right eye showed signs of inflammation in the anterior chamber and bulbar conjunctiva. Pseudomonas was isolated from the conjunctival discharge. Signs of endophthalmitis improved after systemic and topical treatment with antibiotics. The sclera adjacent to the cornea showed signs of melting 2 weeks later. Necritizing scleritis improved after topical and systemic treatment with prednisolone. It showed multiple recurrences after tapering of prednisolone and withdrawal of antibiotics. It subsided after additional treatment with topical tacrolimus for 15 months until present.

Conclusion:Treatment with antibiotics and immunosuppressant was effective for necrotizing scleritis in the present case.
