Abstract Purpose:To evaluate outcomes of cataract surgery in patients with neovascular age-related macular degeneration(nAMD)previously treated by anti-vascular endothelial growth factor(VEGF)agent.

Methods:We retrospectively studied 27 eyes of 26 patients who underwent cataract surgery with a medical history of nAMD treated by anti-VEGF agent previously between 2010 and 2016 at Tokyo Medical University Hospital. Best corrected visual acuity(BCVA)and frequency of anti-VEGF injections were analyzed before and after surgery.

Results:The median follow-up periods and number of injections before cataract surgery were 45.5 months(range:7.9〜85.8 months)and 16(5〜46), respectively. There was no difference in the number of injections given 12 months before and after the cataract surgery(n=20, 5 vs 5). BCVA(logMAR)before cataract surgery was 0.70(0〜1.70)and improved significantly to 0.22(0.18〜1.30)at 1 month after cataract surgery(p<0.001). Twenty months after surgery(n=20), BCVA was stable in 19 eyes(95%)and decreased in 1 eye(5%)due to deterioration of nAMD. Recurrence of macular hemorrhage without visual loss occurred in 1 eye(5%)at 2 months after cataract surgery.

Conclusions:Cataract surgery was beneficial in patients previously treated with anti-VEGF injections for nAMD. However, we should be aware of deterioration of nAMD after cataract surgery.
