Abstract Purpose: To report a case of idiopathic bilateral optic neuritis that responded to plasma exchange therapy.

Case: A 54-year-old female presented with retrobulbar neuritis in the left eye. Visual acuity was 0.01 when seen three days later. Left visual acuity decreased to no light perception 60 days after pulsed corticosteroid therapy. Acute retrobulbar neuritis developed in the right eye 3 months later. As the left eye proved to be resistant to steroid pulse therapy, she was treated by plasma exchange therapy. After the right visual acuity decreased to no light perception, another series of plasma exchange therapy was started 7 weeks after onset. Right visual acuity improved to 1.0. She has been receiving peroral prednisolone and immunosuppressive treatment. Visual acuity in the right eye has been maintained for one year until present. Repeated examinations for anti-AQP4 and anti-myelin oligodendrocyte glyprotein(MOG)have been consistently negative.

Conclusion: Early plasma exchange therapy was apparently effective in a case of corticosteroid-resistant acute retrobulbar neuritis of unknown etiology.
