Abstract Purpose: To report the course until diagnosis in cases of oculomotor nerve palsy secondary to aneurysm in the posterior communicating artery of internal carotid.

Cases: This retrospective study was made on 6 cases who were diagnosed with oculomotor nerve palsy secondary to aneurysm in the posterior communicating artery of internal carotid in the past 10 years.

Findings: Aneurysm in the posterior communicating artery of internal carotid was detected between zero to 7 days after onset of signs suggesting oculomotor nerve palsy. Only one case consulted neurosurgical institution. Only one physician suspected impending rupture of aneurysm in the posterior communicating artery of internal carotid as the cause of oculomotor nerve palsy. The main reason for referral was suspected cerebral infarction.

Conclusion: Rupture of aneurysm in the posterior communicating artery of internal carotid is often overlookd as the cause of oculomotor nerve palsy.
