Abstract. Purpose:We report a case of hypertensive choroidopathy with bilateral bullous retinal detachment. Patient:A 20-year-old man presented with bilateral blurred vision(best-corrected visual acuity, sensus luminis). Bullous retinal detachments and other characteristics of hypertensive choroidoretinopathy were identified in both eyes. The blood pressure was 262/160 mmHg. The patient was diagnosed with malignant hypertension and treated with antihypertension therapy. Optical coherence tomography(OCT)showed a serous retinal detachment(SRD), and a highly reflective fibrin-like accumulation in the subretinal space at the early stage of the disease. After antihypertension therapy, SRD resolved, IS/OS line was partially recovered and fibrin developed into a subretinal fibrotic scar. Results:OCT is useful to understand macular change in hypertensive choroidopathy. Fibrin accumulation in the subfoveal space can lead to severe visual loss.
