要約 目的:悪性高血圧症にみられた両眼胞状網膜剝離の症例についての報告。症例:20歳,男性。両眼の視力低下で当科を受診した。所見:視力は両眼とも光覚弁であり,眼底は網膜動脈の狭細化,網膜出血,網膜深層の黄白色斑,及び上方を除く3象限に胞状網膜剝離を認めた。血圧は262/160mmHgであり,血液検査では著明な腎機能低下,胸部単純X線検査では著明な心拡大と肺うっ血を認めた。悪性高血圧症,急性心不全,急性腎不全と診断し,即日降圧療法を開始,さらに血液透析も施行した。光干渉断層計(OCT)により病初期にフィブリン塊を伴う漿液性網膜剝離を認めたが,経過とともに網膜剝離の消失,フィブリンの器質化,一部の視細胞外節内節の回復を認めた。最終矯正視力は,右(1.0),左(0.1)であった。結論:高血圧性脈絡膜症における黄斑部病変の理解に,OCTは有用な検査手段の1つであり,フィブリン析出と視力予後との関連が示された。
Abstract. Purpose:We report a case of hypertensive choroidopathy with bilateral bullous retinal detachment. Patient:A 20-year-old man presented with bilateral blurred vision(best-corrected visual acuity, sensus luminis). Bullous retinal detachments and other characteristics of hypertensive choroidoretinopathy were identified in both eyes. The blood pressure was 262/160 mmHg. The patient was diagnosed with malignant hypertension and treated with antihypertension therapy. Optical coherence tomography(OCT)showed a serous retinal detachment(SRD), and a highly reflective fibrin-like accumulation in the subretinal space at the early stage of the disease. After antihypertension therapy, SRD resolved, IS/OS line was partially recovered and fibrin developed into a subretinal fibrotic scar. Results:OCT is useful to understand macular change in hypertensive choroidopathy. Fibrin accumulation in the subfoveal space can lead to severe visual loss.
