急激に視力低下をきたした眼窩蜂窩織炎の1例

Sudden visual loss in a case with orbital cellulitis

　43歳，男性。篩骨洞・前頭洞膿嚢腫より波及した眼窩蜂窩織炎により，虚血性視神経症および網膜中心動脈閉塞症を併発し，急激な視力低下をきたした比較的まれな症例について報告した。本症の主訴は，左眼外側部痛と，左眼瞼腫脹であり，眼瞼下垂・眼球突出が見られ，高眼圧を呈していた。視力低下は軽度で，眼底には脈絡膜皺襞を認め，断層撮影にて副鼻腔の含気量低下を認めた。副鼻腔炎と眼窩蜂窩織炎の診断にて抗生物質の大量投与を施行したが，翌日，眼瞼腫脹が増悪した。鼻内篩骨洞開放術を施行したところ，眼瞼腫脹および眼球突出は改善したが視力は0となり，対光反応は消失していた。検眼鏡的には網膜中心動脈閉塞の所見を認めるものの，螢光眼底造影では明らかな色素流入の遅延や閉塞所見は見られなかった。対光反応の出現と共に視力は0.4まで改善したが，視野の障害が残った。

A 43-year-old male developed fulminant cel-lulitis in the left orbit secondary to ethmoid andfrontal sinusitis.In spite of energetic treatmentswith systemic antibiotics and aspiration of pusfrom the lesion, sudden complete visual loss occur-red in the affected eye on day 4 of the disease.Weobserved diffuse retinal edema and optic atrophydue to central retinal artery occlusion in the leftfundus. Elevated orbital pressure and inflammationof orbital tissues seemed to have caused the circula-tory disorder of the orbital artery.
