A 59-year-old male underwent planned extracap-sular cataract extraction in his right eye. Choroidal detachment and severe iridocyclitis developed in both eye 2 months later, leading to the diagnosis of sympathetic ophthalmia.

The patient manifested no systemic abnormal-ities as hearing loss or stiff neck. The symptoms improved with systemic corticosteroids, leaving a 'sunset glow' fundus in both eyes due to marked depigmentation in the choroid. His HLA typing was A24, B51, Bw60 and DRw9. His genetic factor seemed to have participated in the immunological reaction to the iris pigment and residual lens cortex in the eye, resulting in sympathetic ophthalmia.
