両眼性に広範囲に無血管野を認めた再発性硝子体出血の1例

A case of recurrent vitreous hemorrhage with bilateral widespread retinal nonperfusion

　再発性硝子体出血を来し，眼底検査にて両眼性に周辺部網膜に広範囲に無血管野を認めた1例を報告した．

　症例　は33歳女性で，矯正視力右1.2左1.0，眼圧正常，前房，硝子体中には炎症所見を認めなかった．検眼鏡的に左眼硝子体出血，乳頭上新生血管を認めたが，網膜滲出斑は認めなかった．螢光眼底造影にて，両眼乳頭上新生血管，左眼周辺部網膜の新生血管，両眼周辺部網膜の広範な無血管野を認めた．網膜電位図ではb／a比1.0以上，律動様小波の減弱がみられた．両上肢血圧は正常で左右差なく，血液検査等の全身検査では特に異常はみられなかった．アルゴンレーザー光凝固を施行したが，約6カ月間の経過観察では著変は認められなかった．

　現在までに類似の報告は6例のみであり，本症例の6カ月間の経過と非炎症性Eales病との関連につき考察を加えた．

A 33-year-old woman sought medical advice due to recurrent vitreous hemorrhage in her left eye. She had full visual acuity. No inflammatory signs were found in the anterior chamber or the vitreous. Ophthalmoscopy revealed neovascular vessels on the optic disc in both eyes. Fluorescein angiographyshowed the presence of retinal nonperfusion areas over the whole peripheral fundus in both eyes.

Physical and laboratory tests revealed no sys-temic abnormalities. We could find only 6 documented similar cases in literature. We inter-preted this case as a non-inflammatory instance of Eales' disease.
