We evaluated 33 eyes treated with intravitreal sili-cone oil injection as to corneal complications such as band or bullous keratopathy. The cases included 13 eyes of proliferative diabetic retinopathy, 15 eyes with proliferative vitreoretinopathy, 3 traumatic retinal detachment and 2 acute retinal necrosis or Kirisawa-type uveitis.

We divided the eyes into 3 groups according to the location of silicone oil in the anterior chamber; com-plete filling in the anterior chamber (group 1), incom-plete filling with intermittent touch to the cornea (group 2), and silicone oil without touching the cornea (group 3).

Band keratopathy occurred in 7 out of 7 eyes (100%) in group 1, 5 of 7 (57%) in group 2 and none in 19 (0%) in group 3. Bullous keratopathy developed in one (14%) in group 1, 3 (43%) in group 2 and 1 (5%) in group 3. Endothelial damage as well as Newton ring was obser-ved in the vicinity of intracameral silicone oil when seen by specular microscopy. The findings imply that band keratopathy is prone to develop when the anterior chamber is completely filled by silicone oil, due probably to impaired nutrition to the cornea. Mechanical corneal endothelial damage, result-ing in bullous keratopathy, is more likely to occur when the anterior chamber is incompletely filled by silicone oil.
