We evaluated the surgical outcome of a consecutive series of 46 eyes treated by vitrectomy during the past 5-year period. All the eyes necessitated surgery due to vitreous hemorrhage secondary to occlusion of central retinal vein (2 eyes) or its branch (44).

A significant improvement in visual acuity occurred in 39 eyes (85%). In 13 eyes (28%), the final visual acuity was 6/60 or less. The poor visual prognosis was either due to persistent macular edema, preretinal macular fibrosis, tractional or rhegmatogenous retinal detachment.

The surgery was associated with an unexpectedly high incidence of iatrogenic retinal tear (11 %).Postsurgical vitreous hemorrhage occurred in 15 % of the series. All the eyes which rebleeded had not been treated by endophotocoagulation or endodiathermy during surgery.

The findings indicate that meticulous care is neces-sary during vitreous surgery for vitreous hemorrhage secondary to retinal vein occlusion. Endo-photocoagulation or endodiathermy is recommended to avoid postsurgical recurrences.
