斜視手術直後に悪性高熱症を発症し,救命しえた1例を経験した.
症例 は4歳女児で,外斜視のため右眼内直筋前転術を笑気,ハロセン(GOF)麻酔下にて施行したところ本症を発症し,全身冷却,ダントロレン静注などですみやかに処置し救命した.後遺症は認められていない.
眼科領域では,斜視・眼瞼下垂などの小児の外眼筋手術の際に発症することが多いといわれており,特に全身麻酔時には本症の存在を銘記すべきことを強調した.
A 4-year-old female child underwent surgery for exotropia under general anesthesia with GOD. No systemic disorder was detected prior to surgery. At the end of uneventful squint surgery, the body temperature abruptly rose to 39.7℃. This episode of malignant hyperthermia was effectively treated with prompt injection of dantrolen sodium and use of cooling blanket.
Malignant hyperthermia is a possibly fatal com-plication of general anesthesia. Due precaution is to be made as to malignant hyperthermia during eye surgery in children, particularly for strabismus or lid ptosis. This syndrome and its attendant hazards should be known to ophthalmologists.
