ウイルス性結膜炎の臨床疫学的研究—感染経路を中心として

Clinico-epidemiology of viral conjunctivitis with regard to the mode of transmission

　1979年から1982年6月までにウイルス性結膜炎と診断した1,067名について臨床疫学的検討を加えた。院内感染は26.8％〜41.5劣の頻度にて発生し，その主感染源の一つに眼科外来小手術と老人があげられた。家族内感染は新患が再患に較べて多く，流行時に家族内において流行性，非流行性の判断が極めて難しい事を示しえた。プールにおける結膜炎の多くは塩素による刺激症状であり，ウイルス感染ではAd3とAd4によるものがみられ，Ad8によるものは見出す事ができなかった。これはプールでの感染源として眼より大便のウイルス排出を重視する必要を物語るかもしれない。院内感染における老人，家族内感染における乳幼児と母親，プールにおける学童に今後注意する必要があり，眼科専門医としては結膜炎における流行性の患者の選別と共に，サーベイランスにおける情報提供を行う事が大切である。またこのためには臨床所見のみならず病原決定を併行させて行う事が有用であろう。

We evaluated the epidemiology of viral conjunc-tivitis in 1,067 patients seen in eye clinics in the City of Sapporo during 3 years 6 months ending June 1982. The major causative agent was identified as adenovirus type 8 in 1979 and 1980, enterovirus 70 in 1981 and adenovirus type 4 plus enterovirus 70 in 1982.

The incidence of hospital infection fluctuated between 26.8% and 41.5% for each year. Hospital infections chiefly occurred through ocular surgery for hordeolum, chalazion and foreign bodies in the cornea or conjunctiva.
