Lateral brow approachにて全摘出しえた涙腺良性混合腫瘍

A case of benign mixed tumor of lacrimal gland removed totally by lateral brow approach

　CT scanを術前に行う事により，初期の涙腺腫瘍を発見できると共に，腫瘍の大きさ，広がり方（位置関係），骨の変化などを正確に把握できる。これにより，手術方法に対する術前検討が容易となる。

　lateral brow approachにて，涙腺腫瘍に到達し，眼窩縁骨を小さく（3×15mm）切除するだけで，限局した比較的小さい腫瘍は，被膜ごと全摘しうる。このapproachによれば，Krönlein operationと比較して，術後のcomsmeticな面でも，切開創が眉毛に隠れるという利点がある。頭蓋内への穿孔が本法による最も重要な合併症であると思われるが，これに対する注意も，CT scan像で術前検討を加える事によって，十分に可能である。

A 30-year-old male with unilateral exophthalmos of 3 years' duration was diagnosed as lacrimal gland tumor on clinical and CT-scan examinations. We succeeded in the surgical removal of the tumor by the lateral brow approach and by resecting a small piece of superior orbital bone. The tumor proved to be pleomorphic adenoma (benign mixed tumor) on histological studies. The value of CT-scan ex-amination is emphasized in the diagnosis of orbital tumors.
