Fourty-five cases with surgically treated superior oblique palsy were reexamined after an average of 3.6 years. They were classified according to the side of ocular dominance. Postsurgical examinations included vertical deviation, binocular vision and head posture with particular reference to Bielschow-sky head tilt test (BHTT).

Cases with no overaction of the inferior oblique muscle after weakening surgery on the muscle showed a positive BHTT response when the head was tilted towards the affected side. There was no elevation of the eye following recession of the su-peroir rectus muscle.
