上斜筋麻痺の手術療法に関する研究—第1報優位眼側とBielschowsky頭部傾斜試験成績を中心とする上斜筋麻痺の手術療法についての考察—とくに臨床成績について

Surgical treatment of superior oblique palsy.:Part 1. Longterm surgical effects with reference to ocular dominance and Bielschowsky head tilt test

　過去7年間に手術を行った片眼性上斜筋麻痺45名（年齢2〜21歳，平均5歳）を優位眼側により分類し，眼位ずれ，頭位の異常，Bielschowsky頭部傾斜試験（B．H．T．T．）の成績を目標にして，術後1年から7年（平均3年6ヵ月）をわたり検討し，つぎの結論を得た。

（1） B．H．T．T．において，麻痺眼側への頭傾時に麻痺眼が上ひきになるのは，主として，上直筋の働きによることが臨床例で確認された。

（2） B．H．T．T．は，上斜筋麻痺において重要な診断的意義をもつのみならず，頭位異常などの術後の効果判定にも役立つことが判明した。

（3）上斜筋麻痺は優位眼を目標にして分類し，B．H．T．T．の成績とあわせて，その手術適応を考える必要がある。

Fourty-five cases with surgically treated superior oblique palsy were reexamined after an average of 3.6 years. They were classified according to the side of ocular dominance. Postsurgical examinations included vertical deviation, binocular vision and head posture with particular reference to Bielschow-sky head tilt test (BHTT).

Cases with no overaction of the inferior oblique muscle after weakening surgery on the muscle showed a positive BHTT response when the head was tilted towards the affected side. There was no elevation of the eye following recession of the su-peroir rectus muscle.
