眼球内滲潤を伴う悪性リンパ腫の1例

A case of intraocular malignant lymphoma

　両眼視力障害を主訴として眼科を受診し，ぶどう膜炎症状をみた28歳男子で口蓋扁桃部の生検により，悪性リンパ腫（細網肉腫）と診断された症例を経験した。

　悪性リンパ腫の眼球内滲潤に対しては放射線療法が著効を示したが，全身症状は化学療法にかかわらず悪化し，眼科受診後約2ヵ月で死亡した。脳を除く全身臓器の剖検と両眼球摘出が行われ，眼球では主として後極部脈絡膜に腫瘍細胞の滲潤が認められた。

　腸間膜リンパ腺を使っての組織化学的検索を行なった結果，T-cell typeの悪性リンパ腫が疑われた。

A 28-year-old male developed acute bilateral uveitis. The diagnosis of malignant lymphoma (reticulum cell sarcoma) was established after biopsy of swollen tonsils. The ocular condition was refrac-tory to systemic chemotherapy and showed dramatic response to local cobalt radiation therapy. He died 8 weeks after onset of visual loss. Autopsy revealed an extensive involvement of systemic lymph nodes and the viscera. Marked thickening of the posterior choroid was present due to infiltration by tumor cells. The retina and the vitreous were minimally involved in cellular infiltration.
