We preformed 153 penetrating keratoplastic surgery during the 3-year period from 1975 through 1977. We analyzed the factors presumably involved in postsurgical immune reaction and obtained the following results.

Postsurgical immune reaction developed in 73 eyes (47.4%), of which an opaque cornea resulted in 18 eyes (24.7%) accounting for 60% of opaque transplanted cornea. The immune reaction develop-ed within 80 days after surgery in 85% of the cases. Endothelial rejection was the most frequent type of immune reaction (78.2%).
