A 17-year-old boy consulted us complaining of anisocoria and near vision disturbance. His right pupil was mydriatic and did not respond to light and near. The left pupil was irregular in form and sluggish in response to light and near. The instillation of 2.5% mecholyl did not cause miosis bilaterally. The tendon jerks of up-per extremity were hypoactive. Other general neurological examinations were normal.

After 2 months he was aware of excessive swe-ating over the right face. Physical examination revealed that he was anhidrotic except for patc-hy and segmental sweating area of the face and the trunk.
