Kelman水晶体超音波手術装置の使用経験

Cataract extraction with Kelnian phacoernulsifier aspirator. A report of first 113 consecutive cases

　Chales D．　Kelmanは，1962年頃から小切開による有核白内障摘出手術の実験に着手しているが^4,10），水晶体核破砕に超音波振動を使用することを考えつくまでに約3年の試行錯誤を経ている。彼が最初の人眼有核自内障の超音波摘出手術を行なつたのは，1967年^2）であり，最初の症例では核破砕に1時間を要し，手術時間は2時間半にも及んだという。しかしその後は驚くべき短時日のうちに装置，術式の改良がなされ，1968年新潟の臨床眼科学会で彼がその術式の講演を行なつた時には既に現在の装置の基本的機能はほとんど完成されていた。当時は手術の最終段階で水晶体嚢を摘出していたので小切開による全摘出術であつた。その後も装置術式は更に洗練され，実用的な装置がCavitron社から発売された。当初小切開による安静の不要，術後回復期間の短縮などのメリットのみがアメリカのマスメディアを通じて宣伝されたために本法を行なう眼科医のもとに愚者が殺到し，その結果一部の眼科医の中に多分に感情的な反発が起こり，本術式に対する厳しい批判も多く現れた。

The results of first 113 consecutive cases of cataract extraction with the use of a Cavitron/ Kelman Phacoemulsifier-Aspirator have been presented. A total of 54 eyes of 49 cases under-went ultrasonic phacofragmentation and aspira-tion procedure. Other 59 eyes containing 7 eyes in whom the procedure was converted to extracap-sular extraction received aspirations of the soft lens material with the use of I/A tip.

In 49 of the 53 eyes operated on with the KPE procedure 0.5 or better vision was obtained.
